By clicking the submit button, you agree to all terms and conditions of the C19 Sports Camp. Limit of 500 campers will be allowed to participate in said camp. Should camp attendance be full after you submit this form, you will be contacted by a Camp 19 Representative and notified of final attendance status. Camp 19 registration does not guarantee attendance. A parent or guardian must accompany each camper to check-in and pick up the camper at dismissal each camp day. A Release and Hold Harmless Agreement must be signed by a parent or guardian and submitted prior to the beginning of camp. If your child requires medication, a Self Administration of Medication Form must be completed and submitted prior to the camp start. We reserve the right to suspend or terminate any student when it is deemed in the best interest of the camp or the student. The camp is not a school-sponsored program and your school district does not endorse this activity. This camp is no affiliated with your child’s school or the Miami Dolphins football organization or Reizenstein Middle School or Lake Howell High School.
